WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?

We accept any major insurance with out-of-network benefits and Medicare. We are not in-network with insurance allowing flexibility to run our practice in what we believe is the best way.  We focus on treating fewer patients each day so that we can provide individualized high-quality care. Each patient meets one-on-one with a skilled Doctor of Physical Therapy for a full hour, so you get better faster. Your therapist takes the time to evaluate you during EVERY session (not just on day one) so you correct all the underlying problems and progress your treatment appropriately.

Self-pay rates are available upon request.

DO I NEED A DOCTOR’S REFERRAL?

In California, you do not need a physician’s referral or prescription to be evaluated and treated by a licensed physical therapist for up to 45 days or 12 visits, whichever comes first. If treatment is required beyond 45 days or 12 visits, you will need to be referred to a physician for an exam and sign a Plan of Care. If you have a diagnosis from a physician prior to the 45-day/12-visit deadline, you will be allowed visits beyond the time limits. You will need to check with your insurance to ensure that it will cover Direct Access physical therapy treatment. Medicare requires a physician referral within 30 days of the initial evaluation.

WHAT TO EXPECT

During your first 1-hour evaluation we take a medical history, conduct a series of tests and measurements, and discuss your treatment goals and plan of care based on your evaluation. Subsequent treatment sessions are an hour long, one-on-one with a Doctor of Physical Therapy. The number of treatment sessions will vary based on the evaluation.

WHAT IS YOUR NEW PATIENT PROCEDURE?

Schedule your appointment (online booking, phone or email).

 

Provide your insurance information to verify coverage.  

 

Complete and submit the new patient forms at least 24 hours prior to your first appointment.

WHAT TO WEAR

Please wear clothing that allows movement (i.e. tank top or t-shirt) and athletic pants. Wear or bring a pair of shorts for the first appointment. 

CANCELLATION POLICY

Please let us know ASAP if you cannot attend your scheduled appointment. Cancellations within 48 hours result in a $150 cancellation fee.
